eng K-ras Mutation as a Prognostic Factor in Colorectal Cancer Procedures: Laparoscopic vs. Laparotomic Approach Intruduction: The Colorectal carcinoma is the serious problem in The Czech Republic and his incidence has the permanent progress in this time. According to some of the statistic analysies The Czech Republic has the first place among the develop countries. The inserting of the new modalities of the examinational and the therapeutic algorithms which lead to the early diagnosis or to changing of the therapeutic procedures is suitable. The characteraziation of the K-ras mutation: The K-ras gene belongs to the family of ras proto-oncogenes and is conserved by a sequence of eukaryotic genomes. K-ras gene expresses protein p21ras with the function of a G protein. This is a regulator protein of signals initiated by the link between growth factors and cellular surface receptors. Mutations of K-ras proto-oncogenes into an oncogene cancel the internal GTP activity of the p21ras protein, lead to loss of its regulatory function during mitosis, and contribute to the development of malignant tumors, especially carcinoma of the pancreas, large intestine, and lungs. Project objective: The main objective of project is proving the K-ras mutation in the tumors of the colorectum, the detection of the tumors cells with the K-ras mutation in the peripheral blood, the detection of the K-ras mutation in the patients suffering of metastasis of the liver and the verification hypothesis, that the tumors with the K-ras mutation have worse prognosis and that these tumors often lead to the disemination mainly to the liver. Methodology, method of data acquisition: The whole of the project is tied to the grant of The Ministry of Helth of The Czech Republic and runs according to the strict rules of the protocol in The Surgical Dpt. Pardubice and the diagnostic part- the PCR analysis is done in The Biochemical Diagnostic Dpt. Hradec Králové. Preliminary results: The project has been runing from the June, 2004. 56 patients were included in the file with the determine parameters.
