cze Hodnocení interfasciálního tlaku po onkologických operacích na krku. Úvod: Hojení operačních ran po onkochirurgických operacích na krku provází celá řada patofyziologických procesů. V průběhu hojení se vlivem celé řady faktorů, a především zvýšením intersticiálního tlaku, může objevit ischemise tkání na kapilární úrovni, se všemi důsledky v hojení rány. Prevence ischemie rány tkví mimo jiné v pooperačním ošetřování, době trvání podtlakové drenáže atd. Metoda: Pomocí miniaturní silikonové sondy implantované do měkkých tkání krku jsme se pokusili monitorovat tlak ve tkáních. K měření bylo použito Codman microsensor ? čidlo určené původně k implantaci intrakraniálně. Čidlo bylo napojeno na Codman Expres monitor. Nitrotkáňový tlak byl sledován 48 hodin. Závěr: Metoda se jeví jako vhodná pro hodnocení nitrotkáňového tlaku. Vysoká pořizovací cena brání jejímu širšímu využití.

Introduction: The wound healing after the extensive oncosurgical operations of the neck is burdened with the 5% of late wound complications. The syndrom of interfascial hypertension of the neck is one of the numerous factors os wound healing failure. Prevention of the interfascial pressure uncontrolled increase is the monitoring. Material and Methods: The authors use the Codman MicroSensor Basic Kit ? ICP Expres for the monitoring of interfascial pressure. The Tuohy catheter was used for the Codman MicroSensor insertion in to the neck tissues during surgery. The Intracranial Pressure Monitor - ICP Expres was utilised for the measurement. The interfascial pressure was monitored for 48 hours. Results:The method seems to be suitable for the interfascial pressure monitoring in the neck interfascial spaces. The sensors immediately react to the small pressure changes including redresments and vacuum drenage removing. Conclusions:It is only beginning of the use of the method.
