cze Plicní kontuze při hrudním poranění Poranění hrudníku představují významnou kapitolu úrazové chirurgie. Nejčastějším poraněním jsou zlomeniny žeber spolu s pneumohemotoraxem. Zdokonalením vyšetřovacích a především zobrazovacích technik v posledních letech je průkazný vzestup výskytu plicních kontuzí ? 60 % a více. Kombinace poranění hrudní stěny s plicní kontuzí zvyšuje latalitu až na 40 %. Hodnocen soubor 238 zraněných s úrazem hrudníku hospitalizovaných na chirurgické klinice a ARO v letech 2006-2007. Plicní kontuze byla diagnostikována u 147 poraněných (61,8 %), umělá plicní ventilace byla nutná v 53 případech (34,7 %) Léčení hrudních úrazů vychází z hemodynamické stability či nestability zraněného. U plicní kontuze z porušené oxidační a ventilační funkce respiračního systému. Včasná oxigenace, dokonalé tlumení bolesti a v indikovaných případech aktivní chirurgická intervence jsou rozhodujícími články léčby.
eng Lung contusion in thoracic trauma Thoracic trauma is important part of traumatology. The most common injury are the rib fractures together with pneumohemotorax. Improvement of diagnostics methods led to increase of detection of lung contusions by more than 60 %. Combination of thoracic wall injury with pulmonary contusion increases lethality up to 40%. There is assessed the group of 238 patients with thoracic trauma admitted to the Department of Surgery and ICU of Pardubice General Hospital in 2006-2007. Pulmonary contusion was diagnosed in 147 (61,8%) cases, artefitial ventilation was necessary in 53(34,7%) cases. The treatment of thoracic trauma is based on haemodynamic stability of the patient. The treatment of the pulmonary contusion patients is based on impairment of ventilation fnction of the lung. Early oxygenation, analgetics treatment and possible surgical treatment are the most important parts of the treatment. Lung Contusion;Thoratic Trauma
