cze	Flexibilní endoskopické vyšetření polykání - naše první zkušenosti	Flexibilní endoskopické vyšetření polykání (FEES = Flexible Endoscopic Evaluation of Swallowing) je vynikající metoda pro vyšetření orální transportní a faryngeální fáze polykání a částečně nahrazuje skiaskopickou metodu fluoroskopii (MBS = Modified Baryum Swallowing). Tenký flexibilní laryngoskop (průměr 3,4mm) je přes nosní dutinu zaveden na úroveň měkkého patra v nosohltanu. Nejprve je posouzena anatomie, velofaryngeální uzávěr, pohyb a koordinace hltanu a hrtanu a laryngeální addukční reflex. Poté pacient postupně polyká tekutinu (čaj), polotekutou stravu (ovocné pyré) a tuhou stravu (sušenka). Potraviny jsou obarveny potravinářskými barvivy. Celé vyšetření je nehráno do počítače a následně je hodnocena dostatečnost velofaryngeálního uzávěru, snížení hrtanové reaktivity, penetrace do hrtanu, aspirace nebo reflux. Vyšetření se účastní logoped, který pacienta polohuje za účelem usnadnění pasáže sousta a/nebo prevence aspirace. FEES je metoda eliminující rentgenové záření, umožňuje časté opakování vyšetření, vizuální kontrolu posunu sousta a ověření penetrace stravy do hrtanu nebo aspirace. Je nezbytné kvalitní přístrojové vybavení, vynikající znalost anatomie a fyziologie, zvládnutí endoskopické techniky a správná interpretace výsledků vyšetření. eng	Flexible Endoscopic Evaluation of Swallowing - our first experience	Flexible Endoscopic Evaluation of Swallowing (FEES) is a superior method for assessment of oral transport and pharyngeal phase of swallowing and partially alternates skiascopic modified baryum swallowing (MBS) method. Thin flexible laryngoscope (3,4mm in diameter) is passed through the nasal cavity to the level of the soft palate in the nasopharynx. First the anatomy, velo-pharyngeal closure, pharynx and larynx motion and coordination, laryngeal adduction reflex is assessed. Then patient is asked to gradually swallow liquid (tea), semiliquid (fruit purée) and solid bolus (cookie). The food is dyed with food colouring. Complete examination is recorded to the computer and consecutively sufficiency of velo-pharyngeal closure, laryngeal hyporeactivity, laryngeal penetration, aspiration or reflux is described. During the examination speech/language pathologist advises patient to change positions of head and neck to ease the bolus passage and/or prevent aspiration. FEES is a method eliminating X-rays from examination, allowing frequent repetition, permitting visual control of the bolus passage and verification of laryngeal penetration and/or aspiration. Quality equipment, excellent anatomy and physiology knowledge, endoscopic technique skill and proper finding interpretation is limiting for reliable results. It is a method of choice for evaluation of swallowing disorders in proximal parts of swallowing tract.	FEES;Swallowing;Dysphagia;
