cze Perforace ušního bubínku - modelování konečněprvkovým systémem Jedním z příznaků traumatické perforace bubínku je zalehnutí ucha. V závislosti na velikosti perforace nacházíme různý stupeň převodní nedoslýchavosti. K poklesu sluchového prahu přitom dochází především v hlubších frekvencích. Při zhojení perforace se audiogram zpravidla upravuje k původním hodnotám. V experimentu jsme se pokusily vysvětlit akustické děje vedoucí ke zmíněným audiometrickým nálezům. V systému konečných prvků ANSYS byly vytvořeny modely zevního zvukovodu zakončeného: a. celistvým bubínkem b. perforovaným bubínkem Na základě výsledků harmonické analýzy autoři porovnávají rozložení akustického tlaku a akustické výchylky v zevním zvukovodu, na bubínku a ploténce třmínku na zmíněných modelech. Amplitudy akustického tlaku před bubínkem a za ním mají u zdravého ucha podobný průběh. U modelu s perforací bubínku dochází k nelineárnímu zesílení akustického tlaku za bubínkem -v dutině středoušní. Výchylky třmínku byly do frekvence 4 kHz menší a pro frekvence nad 4 kHz větší na modelu s perforací ve srovnání s modelem zvukovodu zakončeného zdravým bubínkem. Modelace potvrzují a blíže vysvětlují audiometrické nálezy u nemocných s traumatickými perforacemi bubínku, především pak skutečnost, že k poklesu sluchu dochází predilekčně na nižších frekvencích. eng Tympanic Membrane Perforation - Finite Element Method Modelation Hearing impairment is one of the symptoms of traumatic tympanic membrane perforation. Degree of conductive hearing loss varies depending on the extent of perforation. Drop in the hearing threshold arise particularly in lower frequencies. The audiogram returns usually to original figures after recovery of perforation. Authors tried to explain acoustic processes resulting in mentioned audiometric findings through an experiment. Models of an external auditory canal closed by: 1. an intact tympanum, 2. a ruptured tympanum, were simulated in the finite element system ANSYS. Distribution of acoustic pressure and acoustic deflections in the external auditory canal, on the tympanic membrane and on the footplate of the stapes in mentioned models is compared. Amplitudes of acoustic pressure in a healthy ear have similar shape of curves before the tympanum and behind it. In the model with ruptured tympanum occurred non- linear increase of acoustic pressure behind the eardrum ? in the middle ear cavity. In the model with perforated tympanum, deflections of the stapes were lesser in frequencies below 4 kHz and bigger in frequencies above 4 kHz compared with the model of external ear canal closed by the healthy tympanum. Models confirm and closely clarify audiometric findings in patients with a traumatic perforation of the tympanic membrane, and above all the fact, that hearing loss is more significant in lower frequencies.
