Typ, intenzita i lokalizace bolestí se různí jak podle typu neuropatie, tak podle patofyziologických procesů vedoucích k neuropatické bolesti. V léčbě diabetické neuropatie a tím i neuropatické bolesti je nutno nejprve pečlivě kompenzovat diabetes a s ním spojené metabolické abnormity. Na podkladě mnoha randomizovaných klinických studií byla prokázána účinnost tricyklických antidepresiv, gabapentinu/pregabalinu, inhibitorů zpětného vychytávání noradrenalinu a serotoninu a opioidů na neuropatickou bolest. Ve 30–40 % má monoterapie dostatečný efekt na neuropatickou bolest, avšak u většiny nemocných je nutno léky kombinovat. eng	Neuropatic pain in diabetic neuropathy	Diabetic neuropathy may be associated with neuropathic pain. Type, intensity and localization of pain differ according to the type of neuropathy or the pathophysiology of neuropathic pain. In treatment of diabetic neuropathy and following neuropathic pain there is necessary to compensate diabetes and associated metabolic abnormalities. On the basis of many randomized clinical trials there was proved effectivity on neuropathic pain in diabetic neuropathy for tricyclic antidepressants, gabapentin/pregabalin, serotonin-noradrenalin- reuptake inhibitors and opioids.
