Hledáli jsme léčebnou metodu, která by nejen snížila výskyt Candidy, ale zmenšila náklady na tuto náročnou léčbu. Tou metodou je preemptivní terapie. Kriticky nemocný pacient je zařazen do skórovacího systému. V tomto systému jsou označeny všechny rizikové faktory u příslušného pacienta. Pacienti dosahující rizikové skóre jsou zařazení do dvou skupin A a B. Pacienti skupiny A byly zajištěni Itrakonazolem, pacienti skupi ny B nedostávali žádná antimykotika. Výsledky studie ukázaly , že preemptivní léčba snížíila výrazně Kandidovou infekci a Kandidemii. Snížení léčebných nákladů bylo zaznamenáno ve specifické skupině rizikových pacientů zajištěných antimykotiky. eng Preemptive Antimycotic Treatment in Critically Ill The incidence of Candida afflictions in critically ill patients is still growing in the Czech Repulic. We looked for a method that not only decreases Candida afflictions but also decreases treatment expenses. This method is preemptive therapy. Critically ill patients were determinated by a risk skore systém . This systém was based on summarizing all the risk factors in on patients. Patients who reach the risk score were randomized into groups A and B. Patients in group A were administered Itraconazole, patients in group B were not administered any antimycotics. Results of the study show that preemptive treatment decreases Candida infections and Candidemia. A decrease of tretment expanses was reached by identifying the specific group of patients at risk using the risk score systém and administering antimycotics.
