eng Vaginal vault suspension according to Williams and Richardson There are several methods to treat vaginal vault prolaps. The most common are suspension of the vaginal vault to the sacrospinous ligament (with or without mesh) and to the sacrum (using abdominal mesh). We present a suspension of the vaginal vault to strips of oblique abdominal aponeurosis. For this, we do a transversal laparotomy. Strips of the oblique abdominis muscle aponeurosis are excised bilaterally, led down laterally to rectus abdominis muscle and behind the peritoneum to the vaginal vault, where they are fixed with permanent sutures. The vaginal vault is covered again with peritoneum and laparotomy is closed again. Special attention must be payed during the passage of the strips in the retroperitoneum not to affect iliac vessels or the ureter. Postoperative complications may arise by ileus when the peritoneum is not closed carefully over strips fixed at the vaginal vault. The abdominal fascia is to be mobilized extensively to adapt it without tension at the end of the operation to avoid abdominal wall hernia. The method presents a stable fixation of the vaginal vault and is recommended especially for prolaps of anterior and apical compartment. vaginal vault prolapse; vaginal vault suspension
